Perform urgent exchange transfusion in consultation with hematology, critical care, or apheresis specialists when there is a rapid progression of ACS as manifested by oxygen saturation of <90% despite supplemental oxygen, increasing respiratory distress, progressive pulmonary infiltrates, the decline in Hb concentration despite simple transfusion, or all of these.
